Care Coordination

Delta Health is participating in the Western Accountable Care Organization (ACO). The Western ACO is a group of doctors, hospitals, and/or other healthcare providers that work together to improve the quality and experience of care you receive. Our hospital chose to be part of an ACO because we think it will help us provide better care to our Medicare patients.

Chronic disease: a growing problem for patients and healthcare professionals

Chronic diseases are the leading causes of death and disability in the U.S., resulting in 7 out of 10 deaths among Americans each year. Multiple chronic conditions negatively affect the quality of life, productivity, and healthcare spending. Extended care beyond the doctor’s office setting is needed to effectively manage Medicare patients with multiple chronic conditions.

Chronic Care Management Program (CCM)
Our Chronic Care Management program for Medicare patients with 2 or more chronic conditions (such as diabetes, high blood pressure, heart disease, etc.) extends care beyond the office setting to help you effectively manage your health.

If you choose to enroll in this program, you will be assigned a dedicated nurse care coordinator who has a deep understanding of patient needs and works directly with you and your primary care provider. The Registered Nurse Care Coordinator will contact you regularly (on the phone or in-person) and can help with medications, doctor appointments, assisting with referral to specialists or outside agencies, answering your questions, or help however you and your provider request.
