MDS/Assessment Coordinator - QA/UR
The MDS/Assessment Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) processes including timely completion/submission of an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care. They will manage the Medicare residents from pre-admission to discharge; develop a care plan including long-term and short-term goals for improvement. The MDS Coordinator will establish and maintain positive relationships with all members of the healthcare team. They are motivated by team building, documentation, and a drive for clinical excellence. Additionally, they will work with and actively participate as a Case Manager, with a special emphasis on the Post Acute Care patient population.
Required Education & Experience:
Must have a current, unencumbered RN license to practice in the state of Colorado (compact acceptable).
2+ years of experience with MDS in a long term care environment is preferred.
Willingness to become RAC-CT certified; experience with Utilization Review is preferred.
Must be able to relate information concerning a patient/resident's condition.
Able to follow all applicable policies and procedures as well as current federal, state and local standards, guideline and regulation that govern swing bed facilities
Must be able to react to emergency situations appropriately when required.
